The game uses blockstate files to define which model is used for each state of a block (for example, stairs use different models depending on wether they are straight stairs, inner corner stairs or outer corner stairs), and then the game uses an item model to define how the block looks in item form, which could be completely different to what the block looks like (for example, fence items look like two fence blocks next to each other, which doesn't happen for placed fence blocks).
